Scheduling of Appointments
All patients (NEW and Existing) can schedule their appointments ONLINE. Please click the link at the top of the page to schedule your appointment. Please note that online appointments can only be scheduled 24 hours in advance. If you require an urgent appointment please telephone the clinic to check availability.
If you are NEW to our clinic, you can download, print and complete your Intake Paperwork prior to your appointment.
New Patient Forms
Ballarat Spinal Health has made Intake Forms available online so they can be completed in the convenience of your own home or office, before you even arrive for your Initial visit.
Please download the necessary form(s), print it out and fill in the required information. Bring this form with you to your appointment OR you can also Email / Fax us your completed form(s).
